Mississinewa was struck inside the “safe” anchorage of Ulithi by a Kaiten manned torpedo. Five Kaitens were launched against Ulithi that morning but only this one caused any damage to American ships. This photo was taken from just over 5 miles away from Mississinewa and yet the black smoke plume is quite prominent.
